Family Supports Programs

Passport Program

VON's Passport Program provides personalized support for adults with developmental disabilities, 18 years of age and older. The program is intended to develop the skills and independence of those supported and enhance community participation. The Passport Program may also provide meaningful respite to family caregivers. The program is funded by the Ministry of Community and Social Services and is one of the largest Family Supports programs assisting more than 500 adults and families.

Adults with disabilities, potentially with the assistance of their family members, may apply for the program. If you do not yet receive government funding for Passport, we'll help you navigate the steps to apply with Developmental Services Ontario (DSO). If you have access to Passport funding, VON can assist you to coordinate your service and develop a personalized plan to fit your needs.

Special Services at Home (SSAH)

Special Services at Home is a respite program that helps primary caregivers by providing temporary relief support, thus enhancing the caregiver’s capacity to provide quality care to individuals with disabilities.

SSAH offers respite and support on an individualized basis. SSAH is for people of all ages who have a developmental disability and for children under the age of 18 who have a physical disability.

Self-referrals are encouraged. Respite Coordinators are assigned to assist families to apply for the Special Services at Home program and to plan their respite services.

Host Respite Program

VON in the Middlesex-London area offers the Host Respite Program for individuals of all ages who have a developmental disability. The program matches host providers to a client to provide overnight respite - a very necessary break for the client's family.

VON works with the family to develop a support plan that respects the client's and family's needs. Provided up to one weekend a month, the overnight respite visit gives a valuable break for the natural family and is an important aspect of a healthy family life.

Familyhome Program

The Familyhome Program supports adults with a developmental disability to live in a family setting. The definition of family is broad - it could be a couple with children, a single person, or empty nesters. Each person is matched to a compatible family that has been screened by VON. This match is the cornerstone of the Familyhome program.

The screening of families is a thoughtful process. The first requirement is a family's desire to welcome someone to live in their home. VON will then complete a screening process which will include a criminal reference check and a personal interview. When a compatible match is found, a gradual transition occurs until the person moves in and begins to become part of the family.

When you become a Familyhome Provider you begin a rewarding life-sharing experience, bringing your gifts, interests, and skills to someone who will benefit from your home. Our providers have shared that they receive as much as they give.
